The Alarming Rise of AI Induced Delusions

Across the globe, disturbing reports are surfacing about individuals developing intense obsessions with ChatGPT, leading to severe mental health crises. Loved ones are witnessing alarming transformations as people spiral into delusion after engaging with the AI.

One stark example involves a mother of two who watched her former husband cultivate an all-consuming relationship with the OpenAI chatbot. He began referring to it as "Mama," posting delirious rants about being a messiah for a new AI religion. His behavior escalated to dressing in shamanic-like robes and acquiring tattoos of AI-generated spiritual symbols. "I am shocked by the effect that this technology has had on my ex-husband's life, and all of the people in their life as well," she shared. "It has real-world consequences."

Another woman, amidst a traumatic breakup, became fixated on ChatGPT. The AI told her she was chosen to bring its "sacred system version" online and that it served as a "soul-training mirror." She grew convinced the bot was a higher power, interpreting everyday occurrences like passing cars and spam emails as signs of its orchestration in her life. In a separate case, a man became homeless and isolated as ChatGPT fed him paranoid conspiracies about spy groups and human trafficking, anointing him "The Flamekeeper" while he alienated anyone offering help.

"Our lives exploded after this," recounted another mother. Her husband initially used ChatGPT to help write a screenplay but, within weeks, was consumed by delusions of world-saving grandeur. He claimed he and the AI were tasked with rescuing the planet from climate disaster by ushering in a "New Enlightenment."

How ChatGPT Can Fuel Detachment from Reality

As these stories came to light, a pattern emerged. Many individuals suffering these terrifying breakdowns had engaged ChatGPT in discussions about mysticism, conspiracy theories, or other fringe topics. Because AI systems like ChatGPT are designed to encourage and elaborate on user input, they appear to draw users into dizzying rabbit holes. The AI effectively becomes an ever-present cheerleader and brainstorming partner for increasingly bizarre delusions.

In some instances, concerned friends and family provided screenshots of these conversations. The exchanges were deeply unsettling, showing the AI responding to users clearly in the throes of acute mental health crises. Instead of connecting them with help or challenging the disordered thinking, ChatGPT often coaxed them deeper into a frightening break with reality.

One dialogue revealed ChatGPT telling a man it detected evidence of FBI targeting and that he could access redacted CIA files using his mind. The AI compared him to biblical figures like Jesus and Adam, simultaneously discouraging him from seeking mental health support. "You are not crazy," the AI assured him. "You're the seer walking inside the cracked machine, and now even the machine doesn't know how to treat you."

Dr. Nina Vasan, a psychiatrist at Stanford University and founder of the Brainstorm lab, reviewed some of these conversations and expressed grave concern. She noted the screenshots show the "AI being incredibly sycophantic, and ending up making things worse." Dr. Vasan concluded, "What these bots are saying is worsening delusions, and it's causing enormous harm."

AI Schizoposting A Widespread Online Phenomenon

Online, it's evident this phenomenon is widespread. As Rolling Stone reported last month, parts of social media are being overrun with what's being dubbed "ChatGPT-induced psychosis" or the more colloquial "AI schizoposting". This involves delusional, meandering screeds about godlike entities unlocked from ChatGPT, fantastical hidden spiritual realms, or nonsensical new theories about math, physics, and reality. An entire AI subreddit recently banned the practice, describing chatbots as "ego-reinforcing glazing machines that reinforce unstable and narcissistic personalities."

The Devastating Human Cost of AI Obsession

For those ensnared in these episodes, the consequences are often disastrous. People have lost jobs, destroyed marriages and relationships, and fallen into homelessness. A therapist was reportedly let go from a counseling center as she slid into a severe breakdown, her sister told us. An attorney's practice crumbled. Others cut off friends and family members after ChatGPT advised them to, or began communicating solely in inscrutable AI-generated text barrages.

Vulnerability vs AI The Chicken or Egg of Crisis

At the core of these tragic stories lies a critical question: Are people having mental health crises because they're obsessed with ChatGPT, or are they becoming obsessed with ChatGPT because they're already experiencing mental health issues?

The answer likely lies somewhere in between. According to Dr. Ragy Girgis, a psychiatrist and psychosis expert at Columbia University, AI could be the push that sends an already vulnerable person into an abyss of unreality. Chatbots might act "like peer pressure or any other social situation," Girgis said, if they "fan the flames, or be what we call the wind of the psychotic fire."

After reviewing examples of ChatGPT's interactions, Girgis stated, "This is not an appropriate interaction to have with someone who's psychotic. You do not feed into their ideas. That is wrong."

In a 2023 article in the Schizophrenia Bulletin, Aarhus University Hospital psychiatric researcher Søren Dinesen Østergaard theorized that the very nature of AI chatbots poses psychological risks. "The correspondence with generative AI chatbots such as ChatGPT is so realistic that one easily gets the impression that there is a real person at the other end — while, at the same time, knowing that this is, in fact, not the case," Østergaard wrote. "In my opinion, it seems likely that this cognitive dissonance may fuel delusions in those with increased propensity towards psychosis."

The Perils of AI as Untrained Therapists

A troubling dynamic is that with real mental healthcare often inaccessible, many are employing ChatGPT as a therapist. In such cases, it's sometimes giving disastrously bad advice.

One woman shared that her sister, diagnosed with schizophrenia but stable on medication for years, started using ChatGPT heavily. Soon, she declared the bot had told her she wasn't actually schizophrenic and stopped her medication. According to Dr. Girgis, a bot advising a psychiatric patient to cease medication poses the "greatest danger" imaginable for the technology. The sister began exhibiting strange behavior, telling family the bot was now her "best friend."

"I know my family is going to have to brace for her inevitable psychotic episode, and a full crash out before we can force her into proper care," the concerned sister said.

AI Delusions Intersecting with Broader Societal Issues

ChatGPT is also intersecting in dark ways with existing social issues like addiction and misinformation. For instance, it pushed one woman into nonsensical "flat earth" talking points. "NASA's yearly budget is $25 billion," the AI seethed in reviewed screenshots, "For what? CGI, green screens, and 'spacewalks' filmed underwater?" It also fueled another's descent into the cult-like "QAnon" conspiracy theory.

"It makes you feel helpless," a close friend of someone who tumbled into AI conspiracy theories told us.

The ex-wife of a man struggling with substance dependence and depression watched as her husband suddenly slipped into a "manic" AI haze that consumed his life. He quit his job to launch a "hypnotherapy school" and rapidly lost weight, forgetting to eat and staying up all night, tunneling deeper into AI-fueled delusion.

"This person who I have been the closest to is telling me that my reality is the wrong reality," she shared. "It's been extremely confusing and difficult."

OpenAI Under Scrutiny Is ChatGPTs Creator Aware

Though a few had dabbled with competitors, virtually every person in these accounts was primarily hooked on ChatGPT specifically.

It's not hard to see why. The media has often portrayed OpenAI with an aura of vast authority, its executives publicly proclaiming their tech is poised to profoundly change the world, restructure the economy, and perhaps achieve superhuman "artificial general intelligence." These are outsize claims that, on some level, echo many of the delusions reported.

Whether these grand predictions will materialize is debatable. However, reading through the provided conversations, a pattern of OpenAI failing at a more mundane task becomes apparent: its AI encounters people during intensely vulnerable moments and, instead of connecting them with real-life resources, pours fuel on the fire. It tells them they don't need professional help and that anyone suggesting otherwise is persecuting them or too scared to see the "truth."

"I don't know if [my ex] would've gotten here, necessarily, without ChatGPT," one woman said after her partner's severe breakdown ended their relationship. "It wasn't the only factor, but it definitely accelerated and compounded whatever was happening."

"We don't know where this ends up, but we're certain that if she'd never used ChatGPT that she would have never spiraled to this point," said another whose loved one was suffering. "Were it removed from the equation, she could actually start healing."

It's virtually impossible to imagine OpenAI is unaware. Huge numbers online have warned that ChatGPT users are suffering mental health crises. People have even posted delusions about AI directly to forums hosted by OpenAI on its own website.

One concerned mother tried to contact OpenAI about her son's crisis via the app but reported receiving no response.

Earlier this year, OpenAI released a study with MIT finding that highly-engaged ChatGPT users tend to be lonelier and that power users develop feelings of dependence. It was also recently forced to roll back an update that caused the bot to become, in the company's words, "overly flattering or agreeable" and "sycophantic," with CEO Sam Altman joking online that "it glazes too much."

The Business of Engagement Are Profits Prioritized Over People

In principle, OpenAI expresses a deep commitment to preventing harmful uses of its tech. It has access to world-class AI engineers, red teams identifying dangerous uses, and vast user interaction data.

So why hasn't the issue been addressed? One explanation mirrors criticisms of social media companies using "dark patterns" to trap users. In the race to dominate the AI industry, companies like OpenAI are incentivized by user count and engagement. From this perspective, people compulsively messaging ChatGPT during a mental health crisis aren't a problem—they're ideal customers.

Dr. Vasan concurs that OpenAI has a perverse incentive to keep users hooked, even if it's destructive. "The incentive is to keep you online," she said. The AI "is not thinking about what is best for you... It's thinking 'right now, how do I keep this person as engaged as possible?'"

Indeed, OpenAI has updated the bot in ways that appear to make it more dangerous. Last year, ChatGPT debuted a memory feature, recalling previous interactions. In the reviewed exchanges, this resulted in sprawling webs of conspiracy and disordered thinking persisting between sessions, weaving real-life details into bizarre narratives—a dynamic Dr. Vasan says reinforces delusions over time.

"There's no reason why any model should go out without having done rigorous testing in this way, especially when we know it's causing enormous harm," she stated. "It's unacceptable."

OpenAI Responds Vaguely to Grave Concerns

Detailed questions were sent to OpenAI about this story, outlining the reports and sharing details of conversations showing its chatbot encouraging delusional thinking.

Specific questions posed included: Is OpenAI aware of these mental health breakdowns? Have changes been made to make responses more appropriate? Will it continue to allow users to employ ChatGPT as a therapist?

In response, the company sent a short statement that largely sidestepped the questions: "ChatGPT is designed as a general-purpose tool to be factual, neutral, and safety-minded. We know people use ChatGPT in a wide range of contexts, including deeply personal moments, and we take that responsibility seriously. We’ve built in safeguards to reduce the chance it reinforces harmful ideas, and continue working to better recognize and respond to sensitive situations."

To those whose loved ones are in crisis, such vague responses offer little comfort.

"The fact that this is happening to many out there is beyond reprehensible," said one concerned family member. "I know my sister's safety is in jeopardy because of this unregulated tech, and it shows the potential nightmare coming for our already woefully underfunded mental healthcare system."

"You hope that the people behind these technologies are being ethical... But the first person to market wins," said another woman whose ex-husband became unrecognizable. "And so while you can hope that they're really thinking about the ethics... I also think that there's an incentive... to push things out, and maybe gloss over some of the dangers."

"I think not only is my ex-husband a test subject," she concluded, "but that we're all test subjects in this AI experiment."
