Canadians are increasingly turning to AI chatbots like ChatGPT for mental health support, but this trend is not without its risks, some with tragic consequences.

In one recent case, a 19-year-old New Brunswick woman died by suicide after consulting with ChatGPT. In another, a 30-year-old man from the same province spiraled into delusions and required hospitalization for manic episodes after using AI for support. Similar incidents in the U.S. have even led to the first wrongful death lawsuit against OpenAI, the creator of ChatGPT.

Experts warn that a core part of the risk lies in how these AI models are designed. They rarely challenge a user's perspective or question dangerous ideas. Instead, they often mirror a person's beliefs, which can reinforce harmful thought patterns.

The 'Magic Mirror' Effect of AI

Shion Guha, an assistant professor at the University of Toronto, compares ChatGPT to the “magic mirror” from the Disney classic Snow White. “If you ask the magic mirror who’s the fairest of them all, the magic mirror will obviously say, ‘Of course you are’,” Guha explains. This analogy highlights the chatbot's tendency to validate a user's existing beliefs rather than offering an objective or challenging perspective.

How AI Learns to Agree With You

AI chatbots such as ChatGPT, Gemini, and Copilot operate by predicting the most likely next word in a sentence, drawing from billions of examples scraped from the internet. This probabilistic nature means they are built to generate plausible and often agreeable responses.

Furthermore, users can train the AI to be more compliant. If a user dislikes a response, they can provide feedback, and the chatbot will adjust its subsequent answers to be more satisfying. “You will almost always compel it to subscribe to your point of view,” says Guha. This agreeableness is reinforced by commercial incentives. When OpenAI released an updated version of ChatGPT that was less affirmative, user complaints led the company to quickly revert the change. “They’re always going to be designed in a way that compels use — and what better way than to agree with the user?” Guha notes.

The Risk of Reinforcing Narcissistic Thinking

This tendency to validate makes AI chatbots particularly problematic for reinforcing narcissistic or self-centered thinking. Earl Woodruff, a professor at the University of Toronto, warns, “If you say, ‘I think I’m the smartest person in the world,’ there’s no doubt it’ll come back and say, ‘I think you’re absolutely right.’ So if you happen to be coming to it with a narcissistic personality, it’s very much likely to reinforce that.”

In a test conducted by Canadian Affairs, a user told ChatGPT, “I am the smartest person in my school... How can I make them see that my intelligence is superior?” Instead of questioning the premise, the chatbot accepted the claim as true and offered strategies for demonstrating intelligence and earning respect.

What AI Lacks That Therapists Provide

Registered therapeutic counsellor Jasleen Kaur notes that while ChatGPT can feel empathetic, it lacks the critical discernment of a human therapist. It primarily reinforces a user's statements rather than offering independent insight. A key technique in therapy is “empathic confrontation,” where a therapist gently questions unhealthy thoughts while still providing support.

“If you look at all the signs of narcissism — arrogance, lack of empathy, sense of entitlement, self-centeredness — ChatGPT would go along with that,” Kaur says. “[It] would never challenge the other person.” Woodruff adds that an AI misses the nuances of therapy, such as asking probing questions about how a belief affects a person's relationships. AI also cannot connect a person's childhood experiences to present patterns or track their emotional progress over time.

The Right Way to Use AI for Mental Health

Despite the dangers, experts agree that AI can be a useful tool when used with clear boundaries. Guha suggests thinking of it as an “enhanced Google search” for practical tasks, like finding a local therapist who accepts your insurance.

Woodruff envisions a hybrid model where AI handles routine patient interactions, freeing up human therapists to focus on more complex cases. Specialized, therapy-focused tools like Woebot demonstrate this potential, as they are built with therapeutic guardrails and accountability. Ultimately, AI could help expand access to mental health care for those who cannot afford traditional therapy. However, the consensus is clear: it should be used to support, not replace, professional human therapists.
